A Note from the Nurses
Sunny greetings, and a warm welcome to our new families! As we prepare for another school year, I'd like to share a few important health reminders to ensure your student is ready for the first day of school.
Immunization Requirements
Parents and guardians of Kindergarten, 7th grade, and 12th grade students are reminded that proof of current, up-to-date immunizations is required. Thank you for submitting your student's immunization records to the school as soon as possible if you have not already done so.
Rescue Medications
If your student requires a rescue medication for asthma, severe allergies, diabetes, or seizures, please bring the medication, Medication Authorization Form and Emergency Action Plan on or before the first day of school. Medication and forms may be dropped off at the Main Office or stop by and see us at Knights on the Lawn!
Please note that we cannot accept or administer rescue medications without accompanying paperwork completed and signed by your student's medical provider.
Daily Medications at School
If your student needs to take medication during the school day, please complete the Medication Authorization Form and bring:
- The completed medication authorization form
- The medication in its original pharmacy-labeled container
We are unable to administer daily medications without the completed authorization form.
Medication Authorization forms and Emergency Action Plans can be found on our Health Services page and are valid for 13 months from date signed. If you have any questions or need assistance with any of the required forms, please reach out! We look forward to seeing you soon!
